Job Summary
We are seeking a compassionate and dedicated Social Care Worker to join our team. The ideal candidate will be responsible for providing support and assistance to individuals and families. This role involves assessing client needs, developing care support plans, and connecting clients with appropriate resources. Providing 1:1 support will play a crucial role in advocating for clients' rights and ensuring they receive the necessary service to improve their well-being.
Duties
1. Help to develop, implement, and monitor individualised care plans.
2. Provide care and support to clients facing emotional, social, financial and/or health difficulties.
3. Collaborate with other professionals, including healthcare providers and community organisations, to coordinate services.
4. Advocate for clients’ rights and access to necessary resources.
5. Maintain accurate records of client interactions and progress.
6. Participate in reviews and multidisciplinary team meetings.
7. Mentor colleagues in services as required.
Skills
1. Strong interpersonal skills with the ability to build rapport with clients from diverse backgrounds.
2. Excellent communication skills, both verbal and written.
3. Proficient in problem-solving and critical thinking to address complex client issues.
4. Ability to work independently as well as part of a team.
5. Knowledge of relevant legislation, policies, and best practices in social care work.
6. Empathy and a non-judgemental attitude towards clients.
