JobSummary:
To dischargethe Authority s responsibilities under ASCOF and other relevantlegislation with respect to Service Users with disabilitiesvulnerabilities and Eligible careneeds.
To ensure thecouncil s eligibility criteria policies and procedure are followedin relation to serviceusers.
To help createpackages of support to prevent unnecessary reception of people intocare and to assist people to remain in their own home wherepossible and/or to move to suitable and appropriate residentialcare.
To matchservice user needs to potential placements orproviders
KeyDuties/Accountabilities:
To receivereferrals and assessments from care managers and social workers forService
Users requiring services in atimely professional efficient and positive manner. To assess theneeds and desired outcomes and to match these against anappropriate provision in order identify the best value placementfor the ServiceUser.
To makerecommendations to the social worker and on the most appropriateand best value options that will meet the service user s needs. Totake responsibility for ensuring that the provider/organisation isboth fit for purpose and able to meet the needs of the service userincluding undertaking reference registration and evaluation checkson the providers ifrequired
To ensure whererequired that notifications of service changes are made to theFinance department to ensure that where required financialassessments are undertaken of the Service User for calculation ofcontribution to the cost of the carepackage
EssentialExperienceRequired:
Experience ofusing computer packages and undertaking administrativetasks.
Experience ofnegotiating agreements orcontracts
Previousexperience of a customer focussedenvironment
Evidence ofcontinued professional and personaldevelopment
EssentialQualificationsRequired:
Educated to ahigh level A levels as aminimum
Progression toBR10 would be through the appraisal system and the criteria asbelow
Additionalinformation tonote:
Occasionalweekend work may be required (4hrs sat /sun)
Rota will beMonday and Tuesday on Week one and then Tuesday Wednesday andThursday on Week two. On average 18 hrs p/w or 36 across the 2weeks
