Social Worker - Cared for Team
We are looking for a Social Worker to support young people and adults as they transition from children's services into adult care. This role is crucial in ensuring individuals experience a smooth and well-supported transition, enabling them to achieve independence, stability, and the best possible outcomes.
Key Responsibilities:
1. Conduct comprehensive assessments and create tailored transition plans.
2. Provide guidance and advocacy to young people and their families throughout the process.
3. Collaborate with multidisciplinary teams, including education, health, and social care professionals.
4. Ensure compliance with safeguarding policies and care legislation.
5. Maintain accurate records and provide detailed reports to inform decision-making.
6. Promote independence, inclusion, and person-centered care approaches.

About you:
* Hold a social work degree
* Right to work in the UK and hold a clean UK driving license
* Registered with SWE
* Post-qualification experience within children and family services
