The Older people's services work within the Acute Response Team providing specialist assessment and interventions for patients admitted with a new event impacting on their ability to engage in the occupations that are important to them, supporting them to regain their independence or find ways to manage activities that are important to them to be able to leave the hospital. You will have a passion for supporting older people to live their best life and be an essential part of the MDT. Main duties of the job As a Band 6 Occupational Therapist, you will provide high-quality, patient-centred care to older adults in an acute setting, supporting their independence and facilitating timely hospital discharge. You'll work collaboratively with Front Door Frailty Teams, Hospital at Home services, and wider multidisciplinary teams to ensure a seamless and efficient patient journey.
