Job summary We are seeking to recruit a full time Community Practitioner to our South Northumberland Community Treatment team which supports clients in the areas of Cramlington and Blyth. Via our ongoing service improvement models we seek to recruit a suitably qualified person who is dynamic and motivated with a specific interest in the provision of top quality secondary level mental health care within the community. The successful applicant will have appropriate knowledge and skills to assess, deliver, and coordinate treatment and adhere to local and national targets and NHS drivers. As a band 6 Community Practitioner you will also be required to offer supervision and support to other members of the team. The successful applicant will possess good communication and interpersonal skills, be focused and well organised and have the ability to be flexible and responsive as well as demonstrating a positive, caring and compassionate attitude to this role. Effective clinical skills, as well as the ability to stay motivated and the willingness to be creative in a stressful environment are a must. Main duties of the job To be responsible for the Assessment and Management of casework of service users who have substantial and complex needs. Provide specialist clinical advice to Service Users/Carers, staff, statutory agencies, 3rd sector partner agencies and the public. In collaboration with the Service User and Carer where appropriate, develop Recovery and Wellbeing focused Care Plans, ensuring regular reviews to monitor progress and adapt Care Plan as necessary. To consider Carer needs and ensure Carer and family are involved in Care Planning where appropriate To deliver evidenced based Clinical and Psychologically based Interventions. The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
