This exciting role will make a real difference, promoting the uptake of annual health checks for patients registered with Serious Mental Illness (SMI) across all our 19 GP Practices in Calderdale. Building on existing efforts this role will also support practices in Calderdale to in deliver the highest quality SMI Health Checks for their patients. Working closely with our GP Practices, Mental Health Practitioner roles, Personalised Care Teams, VCSE and patients registered with an SMI promoting personalised care and addressing health inequalities. Main duties of the job Promote both the quantity and quality of SMI Annual Health Checks by working with agencies including primary care, secondary care, local authority and VCSE teams across Calderdale to build on and embed existing work in this field. Identify and address health inequalities off those with an SMI accessing general practice. Including the identification of barriers and working with partner agencies to address them including but not limited to connections with prisons and housing. Work to close the loop of a health check, ensuring checks lead to support and interventions. To represent PGPA at Emotional Health & Wellbeing Alliance meetings. An understanding of the complexities of health conditions and the barriers to accessing services for individuals with complex mental health needs/SMI. Awareness of managing strategies for change within a workplace setting.
