The Community Learning Disability Team has a vacancy for a Band 6 Nurse. This role will support people with a learning disability who are placed out of area and work with people in the community who require nurse input. The Learning Disability Team is a multi-disciplinary team who are passionate about providing the best possible care. Main duties of the job
1. Responsible for providing care, interventions, and health promotion to people with learning disabilities.
2. Promote robust communication networks with clients, carers, and other health workers and care agencies.
3. Utilise a range of communication skills to ensure that clients with learning disabilities understand the implications of their health issues, including complex health problems.
4. Assist capacity for consent to treatment and provide advice regarding best interests when a client lacks capacity.
5. Maintain a knowledge/skills base and evidence the same through a personal professional portfolio and act in accordance with the NMC code of professional conduct.
6. Undertake a supervisory role in respect to multi-disciplinary junior team members and work alongside the Team Leader on clinical issues such as therapeutic approaches and care co-ordination.
7. Lead on supporting people with a Learning Disability who have been placed out of area.

Qualifications
* RNLD or RNMH with live NMC registration
* Evidence of on-going development and post registration training
* Evidence of ongoing professional development related to Learning Disabilities
* Demonstrable Care Co-Ordination skills
* Training in Risk assessment and management skills
* Experience in the assessment and management of people with moderate/severe Learning Disabilities who present with co-morbidities
* Evidence of undertaking audit work
* Innovative, creative, and a willingness to try new ideas
* Completion of further clinical training
* Completion of leadership training
