Participate in patient satisfaction reporting to improve patient care Planning and Organising Skills Responsible for the provision of appropriate, high-quality care to a defined caseload. Responsible for workload planning. Undertake a holistic assessment of need for the child/young person (YP) and their family, using the appropriate model of care and evidence-based practice. Participate in the development, implementation, and monitoring of the clinical team contract/objectives. Provide information to the Continence Team Lead and Community Matron on patient activity as required to promote effective service management Accurately record & maintain health records in line with NMC & Trust policy Maintain up to date knowledge and competence in line with professional and service requirements, mandatory training, in line with Trust Policy and demonstrate critical thinking, decision-making and reflective skills to ensure own professional development. Maintain registration in line with professional bodies.
