Hospital Liaison Nurse Field-based (UK) with occasional travel to client offices and meeting venues Competitive, plus excellent benefits About the Role We are looking for a compassionate and dedicated Hospital Liaison Nurse to join our Patient Solutions team. In this field-based role, you will facilitate seamless transitions for patients from hospital to home, ensuring high-quality care and strong relationships with NHS hospitals. You will provide both in-person and virtual clinical support to patients, families, and healthcare professionals. Key Responsibilities Deliver safe, patient-centered care during hospital-to-home transitions. Collaborate with hospital teams to develop care packages for discharge. Communicate effectively with multi-disciplinary teams. Provide clinical care following program guidelines, both in-person and virtually. Educate and support patients, families, and carers. Maintain accurate patient data and report changes in care. Ensure compliance with regulatory requirements and client-specific training. Requirements & Qualifications Current NMC registration and degree-level education (or equivalent). Minimum 2 years post-registration experience. Additional qualifications in CVAD, Parenteral Nutrition, or related fields (desirable). Full UK driving licence. Strong attention to detail, communication, and digital platform skills.
