Care Navigation: Direct patients to the most appropriate healthcare services based on their needs, using care navigation protocols. Ensure patients are informed about available services and how to access them, both within the practice and externally. 3. Patient Interaction: Engage with patients to understand their needs and provide guidance on accessing the right care. Address patient queries and concerns, ensuring a positive and supportive experience. 4. Collaboration: Work closely with GPs, nurses, and other healthcare professionals to coordinate patient care. Participate in team meetings and contribute to the development of care navigation strategies. 5. Administrative Support: Assist with administrative tasks, including updating patient records and managing correspondence. Ensure all documentation is accurate and complies with practice policies and procedures. 6. Communication: Maintain clear and effective communication with patients and the healthcare team. Provide information and updates to patients regarding their care and appointments.
