East Kennet Primary Care Network (PCN) are looking for a Clinical Pharmacist to join our team and support our practices.
We are a friendly & approachable team with an eagerness to support our patients within the community whilst improving our quality and safety of care.
Salary will range from £38,000 - £47,000+ (depending on experience and qualifications).
We are open to considering Full-Time or Part-Time positions to support a wider team dynamic.

Main duties of the job
The Clinical Pharmacist will improve the quality and safety of care offered to our patients across the PCN and reduce GP workload.
The Pharmacist will support the practice to carry out medication reviews, support patients with long term conditions, reconcile hospital discharge summaries, answer patient queries and audits.

The East Kennet Primary Care Network, which consists of four neighbourhood GP practises with a total patient list of 38,000, is now a well-established network. We are situated in East Kennet, Wiltshire - a beautiful rural region of the UK.
The GP surgeries within the PCN are Kennet & Avon Medical Partnership, Ramsbury & Wanborough Surgery, Old School Surgery Great Bedwyn, and Burbage Surgery.
Job responsibilities
As the Clinical Pharmacist your responsibilities will vary. You will:
* Ensure the provision and delivery of an integrated, safe, legal, high quality pharmaceutical service.
* Provide expert advice on pharmaceutical matters and ensure a safe and modern medicines management services.
* Work independently providing a service that includes medication reviews, repeat prescriptions, managing long term health conditions, medicines reconciliation, audits, and answering patients' medication queries.
This role will support the following practices: Ramsbury & Wanborough; Old School Surgery, Bedwyn, and Burbage Surgery.
Person Specification
Experience
* Experience with working as part of a team and actively supporting patients in various healthcare settings.
* Experience of working within a General Practice setting.
Registered Pharmacist
* GPhC Registered Pharmacist.
