Job summary
Plan to provide high quality care, you will work within a small team delivering Continuity of Care to young parents and their babies throughout pregnancy and in the postpartum period.
Recognising the benefits of the continuity of carer models and the delivery of safer and more personalised maternity care, the MSW will undertake a range of delegated clinical and clerical duties within agreed guidelines and protocols and under the direction and supervision of the midwife.
Main duties of the job:
* Supports the young parents' maternity team in providing continuity of care throughout pregnancy and the postpartum period.
* Develops, manages and maintains positive, appropriate relationships with young people and their families demonstrating respect, kindness, compassion and empathy always.
* Uses language and approaches that are relevant and respectful to young people.
* Understands the potential social, emotional, and family dynamics that young parents may face.
* Builds trust and rapport with young parents to provide effective support.
* Consistently employs strategies to promote personalised, informed decision-making when planning, implementing and evaluating care activities.
* Understands the importance of both the physical and mental well-being of young parents and their families. Knows how to refer to appropriate interventions.
* Tailors information to meet the needs of individuals, including where complex care and support needs exist.

Job responsibilities:
* Work within the young parents midwifery team to provide personalised care and support to the young people and their families within the team's caseload.
* Provide care directly, in and out of hospital including practical and emotional support demonstrating excellent communication skills and sensitive care to address the unique challenges of teenage parenthood.
* Support the delivery of age-appropriate antenatal education programmes with a focus on the promotion of confidence, teaching parents how to care for their baby and increase belief in early parenting skills either one-to-one or in groups.
* Assist young parents to access necessary support services such as housing assistance, educational opportunities, and mental health counseling.
* Work closely across multi-disciplinary teams to optimise a package of support tailored to the young person's needs, including but not limited to health visiting services, youth services, community support services, mental health practitioners, benefits, social care and other key peer-support workers.
* Provide public health and health promotion initiatives.
* Act as a point of contact for support or signposting.
* Demonstrate a high degree of technical proficiency while undertaking tasks and activities.
* Document appropriately, detailing actions taken, where support has been sought from suitably qualified practitioners, or where concerns have been escalated.
* Evaluate own and others practice and suggest and implement changes to improve service delivery.
* Engage in audits or surveys to support service improvement.
* Demonstrate good communication and organisational skills.
* Independently prioritise and plan own workload.
Person Specification Qualifications & Education Essential:
* Care Certificate
* 5 GCSE's Grade C/5 including English and Math's or equivalent qualification
* NVQ 2 or equivalent and commitment to development within role
Desirable:
* Evidence of completion of MSW Band 3 competencies
* NVQ L3 or equivalent and commitment to development within role
Knowledge & Experience Essential:
* Previous experience in a similar role
Desirable:
* Previous experience as a band 2 HCA or Support Worker
* Experience of working with new-born infants
