LSCFT Specialist Mental Health Rehabilitation is currently undergoing transformation across inpatient and community services in order to implement a robust pathway for people with complex mental health needs in the least restrictive and safest way, there is a need to develop the range of service provision that will enable this.
The aim of the transformation is to provide a service which can offer a whole pathway of support locally, identified in both national plans and clinical guidance, which would include an inpatient and community service offer that is matched against the identified need of LSCFT population. In order to achieve our aims LSCFT MH Rehab needs to standardise the approach to the provision of these services; to identify and reduce inequalities and improve quality, ensure services are safe, effective, evidence based and informed.
The MH Transformation programme of work ensures we are working towards an end to end service pathway that will support people with the most complex mental health needs at the right time and in the right place.

An exciting opportunity for a band 6 Nurse Team leader Nurse to join our transforming Community Prevention and Engagement Team (CPET) for a highly skilled practitioner to work across Fylde and Wyre working with an array of mental health services and key stakeholders this team holds the importance of social inclusion at its heart. Working from ideology that people achieve a more sustained recovery when community connection is apparent this is a team that comprises of skilled and specialised support staff working across all mental health pathways to assertively engage and support individuals in working towards therapeutic recovery., Lancashire & South Cumbria NHS Foundation Trust provides a range of services including secondary mental health care across this area. Specialist provision comprises inpatient child and adolescent mental health services, perinatal mental health and forensic services including low and medium secure care.
The Trust also provides a range of physical health and well-being services in the community alongside a range of partners in the Lancashire, Cumbria and Sefton area.
