We are excited to have an opportunity for a clinical nurse specialist to join our established team in Bury St Edmunds. You must be able to demonstrate a warm, compassionate and needs-led approach to working with our service users. As part of this team, responsibilities would include undertaking audits whilst promoting quality care, working collaboratively with individuals and other supporting agencies to manage needs on discharge from hospital as well as leading the team by example to demonstrate high quality care., This role will give you the time and facilities to make a difference. You will have the ability to offer and support the team with person-centred care whilst building trusting relationships to increase engagement with individuals suffering from severe mental illness during the admission process. The main duties are:
* Monitoring patients mental state and to support formulating working diagnoses, with effective treatment plans, developed by the MDT.
* Working alongside the MDT to promote pharmacological and non-pharmacological treatment plans for a range of mental health conditions such as psychotic disorders, affective disorders, insomnia, personality disorders and anxiety disorders and organic diagnosis.
* Monitoring the effectiveness of assessment and treatment
* Evaluate and promote the writing of collaborative care plans, support discharge plans and provide assurance of the quality of care that is offered to the older adult.
* Work collaboratively within and across the MDT and other agencies.
* Manage and hold responsibility of complex cases.
* Clinical leadership for the staff within the ward team
