Job overview
The Breast CNS is an integral part of the Breast cancer nurse specialist team and provides patients with specialist support and advice. The post holder will have ongoing responsibility for the coordination of patients and ensuring that they are given the right support throughout the cancer pathway.
The Breast CNS will deliver support at diagnosis and continue to support the patient and their carers either face to face or through designated nurse-led telephone clinics.
Main duties of the job
1. Support the patient at diagnosis and ensure that they have the contact details for the breast care nurses.
2. Assess and identify the needs of cancer patients via Holistic Needs Assessment; ensuring that when appropriate, referral to other members of the multidisciplinary teams are made such as social workers, counselling, and/or psychiatric/psychosexual help.
3. Ability to assess and triage patients who have called with a concern and ensure that appropriate advice and support is given if required.
4. Ability to communicate highly sensitive information - i.e. results of tests and investigations - to patients and carers in a clear, compassionate, and empathetic manner. Assess patients’ needs and barriers to understanding information and provide continuing information and support to patients and their carers during treatment, whether inpatient or outpatient.
5. Use of the Trust's IT Infoflex systems to ensure robust tracking of mammogram, communication, and treatment of patients.
6. Provide communication and collaboration between all healthcare professionals both in the Hospital and Primary Care setting, including support agencies.
7. Liaise with members of the Multidisciplinary Team in the care and treatment of cancer patients to optimise health outcomes.
8. Act as the patient's advocate at all times, in particular in MDT discussions ensuring that clinical decisions are made.

* NMC Registered Nurse with over 2 years post qualification.
* Experience caring for patients and families with breast cancer.
* Able to work autonomously and provide evidence of this.
* Awareness of the key points in the Cancer Pathway.
* Demonstrates effective communication skills.
* Experience in clinical documentation.
* Degree in Nursing.
* Advanced Communications Skills Training.
* Awareness of the breast multidisciplinary team meetings.
* Awareness of the Holistic Needs Assessments.
* Evidence of working well in a team.
At interview
* Registered Nurse NMC.
* Experience working with Breast Cancer Patients.
* Ability to work in a team and autonomously.
