Community Advanced Health Care Assistant

Come and join an exciting service within North Lincolnshire who support patients when leaving hospital that require short term care and rehabilitation. The service will provide care support and re-enablement for patients and their families across seven days a week. The team will work in collaboration with the District Nurses, locality coordinators, discharge teams and will closely link with community therapy services to promote functional independence and rehabilitation for short term care and support needs. The advanced community health care will undertake administration of medication and must have appropriate training to be able to do so. Training will be provided in line with local policy where this is not achieved at application stage.
Please note that evidence of the NVQ qualification will be required on the day of the interview.
Main duties of the job
The role will involve assisting with patient's personal care and support needs up to 4 visits a day over 7-days a week. The role will involve caring for patients who have recently been discharged from hospital and are returning home with a change in care and support needs. With compassion, the successful applicant will be expected to nurture independence wherever possible to promote improved quality of life. The successful applicant will care for the individual physically with respect and promote dignity throughout. The successful applicant will respect the individual's wishes and secure consent for their activities. The successful applicant must be motivated to provide a high standard of care, adaptable to the individual and family's needs, and be resourceful when needed.

Education and Qualifications
* Must hold NVQ or NQF equivalent level 3 training in health care and social care
* Has undertaken some recent study
Occupational Experience
* Experience of working in a health or social care setting for 6 months
* Holds a driving license
