Job Summary:
The Care Coordinator is responsible for planning, coordinating, and overseeing the delivery of high-quality care services to individuals in need. They work closely with clients, families, healthcare professionals, and social services to ensure appropriate care plans are implemented, monitored, and adjusted as needed.
Key Responsibilities:
Care Planning & Coordination:
* Develop, implement, and review individual care plans in collaboration with clients, families, and multidisciplinary teams.
* Assess clients' needs, preferences, and risks to ensure the provision of person-centered care.
* Coordinate care services, ensuring timely and effective delivery of support.
Client & Family Support:
* Act as the primary point of contact for clients and their families, providing information, guidance, and emotional support.
* Address any concerns or issues raised by clients or their families promptly and professionally.
Liaison with Healthcare & Social Care Professionals:
* Work closely with doctors, nurses, social workers, and other healthcare professionals to facilitate seamless care delivery.
* Arrange and coordinate external services such as therapy, specialist consultations, and community resources.
Compliance & Documentation:
* Maintain accurate and up-to-date records of care plans, assessments, and communications.
* Ensure all care services comply with relevant regulations, policies, and best practices.
* Participate in audits, inspections, and quality assurance processes.
Team Support & Supervision:
* Provide guidance and support to care staff, ensuring they deliver high-quality care.
* Conduct training sessions and team meetings to enhance staff knowledge and performance.
* Monitor staff adherence to care plans and professional standards.
Crisis Management & Problem-Solving:
* Respond effectively to emergencies, safeguarding concerns, and unexpected care issues.
* Implement appropriate solutions and escalate concerns when necessary.
Qualifications & Experience:
* Essential:
o Experience in health and social care, particularly in care coordination or case management.
o Strong understanding of care planning, safeguarding, and person-centered approaches.
o Excellent communication and interpersonal skills.
o Ability to manage multiple tasks and prioritize effectively.
o Competency in using care management software and record-keeping systems.
* Desirable:
o NVQ Level 3 or 4 in Health & Social Care or equivalent qualification.
o Knowledge of relevant healthcare regulations and best practices.
o Experience in leading or supervising a care team.
