SummerhillSurgery is a well-established local GP surgery in the Kingswinford area.
Weare looking for an additional member to join our Practice Nursing team due toan increase in registered population.
The post holder is responsible for thedelivery of basic practice nursing services care to the practice population.Supported by senior nurses within the Practice, they will deliver care withinthe boundaries of their role, focusing upon supporting patients to be healthy,monitoring of long-term conditions, health prevention and screening activities.
Theywill work collaboratively with the general practice team to meet the needs ofpatients, supporting the delivery of policy and procedures and providing nurseleadership as required.
Main duties of the job
Assist with the organisation andco-ordination of the provision of nursing services for the practice.
Provide nursing treatment to patients inparticipation with general practitioners or independently agreed protocols,including cervical smears.
Provide general and specific health screeningto the practice patients (within the agreed protocols) with referral to generalpractitioners as necessary.
Be competent in administration of childhoodimmunisationand travel vaccination.
Be competent and confident in managing peoplewith long term health conditions
Assess, diagnosis, plan, implement and evaluateinterventions/treatments for patients with complex needs Proactively identify,diagnose and manage treatment plans for patients at risk of developing along-term condition (as appropriate)
Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care
Support patients to adopt health promotion strategiesthat promote healthy lifestyles, and apply principles of self-care.
Implement and participate in vaccination andimmunisation programmes for both adults and children
Meet the needs of patients presenting foropportunistic wound care

Summerhill Surgery is a happy surgery trying toprovide the best possible service and have a patient focused ethos.
We currently have 7300 patients with approximately15% living in Care Homes who are looked after by our specialist team.
We have 3 GP Partners and a non-clinical ManagingPartner with 3 further salaried GP's. Other members of our Clinical teaminclude ANPs, Nurse Practitioners, Practice Nurses, HCA's, a GP Assistant, aParamedic and Pharmacist support. We also have a large hardworking admin teamproviding support to the clinicians and patients.
Job responsibilities
To work as part of the team responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice including care for elderly and housebound patients.
Working as a senior member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the entitled patient population.
The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, health promotion, chronic disease management, health promotion, well women and well man clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure.
This organisation will ensure that the post holder has access to appropriate clinical supervision and support on a day-to-day basis.

Qualifications
* A Registered Nurse
* Experience working in a primary care environment
* Experience working autonomously
* Understanding the importance of evidence-based practice
* Ability to record accurate clinical notes
* Ability to work within own scope of practice and understanding when to refer to GPs
* .
* Clinical knowledge in the following-
* Wound care/removal of sutures and staples
* ECGs
* Venepuncture
* New patient medicals
* Immunisations (routine, childhood and travel)
* Requesting pathology tests and processing the results, advising patients accordingly
* .
* Chronic disease management awareness for-
* Diabetes
* CHD
* Asthma
* Specialist interest in at least 1 Chronic Disease, e.g. Asthma, COPD or Diabetes
* Experience of managing elderly and housebound patients
* Experience using the EMIS clinical system
* Experience with clinical risk management
