Job summary The Bone Health & Fracture Prevention Clinical Nurse Specialist (CNS) will provide specialist support for patients over the age of 50 who sustain a fragility fracture by coordinating the Fracture Liaison Service forWeston General Hospital. Aspects of this work will include:-Identifying patients who have recently suffered a low impact (fragility) fracture.-Referring patients under the age of 75 for assessment of bone mineral density (a Dual Energy X-ray Absorbtiometer ( DXA) scan) in line with NICE TA161.-Communicating the need for bone protection therapy and calcium/vitamin D3 supplementation to GP's ofpatients over the age of 75,without the need for a DXA scan, unless there a reasons for DXA scanning (as per protocol).- Identification of patients who will benefit from an osteoporosis specialist clinic review with either Rheumatologist or CNS.-Identification and coordination of patients requiring initiation on parenteral treatments eg Denosumab or IV bisphophonates or Teriparatide-Discussing DEXA results, treatment options, life style advise and pain management to patients in outpatient appointments (after training)-Provide education, training and health promotion sessions to patients and health professionals.-Carrying out audit programmes to establish effectiveness of fragility fracture patient identification, referral pathways and treatment interventions.-Involvement in Research and Development research studies appropriate to department.
