Job summary
As a remote Clinical Pharmacist you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and pharmacy technicians.
You will help resolve medication-related queries, work as part of a multidisciplinary team, improve patient outcomes and support the improvementof access to healthcare.
Main duties of the job
1. As a clinical pharmacist you will confidently act within professional boundaries, supporting and working alongside a team of pharmacists and GP's.
2. Resolve medication-related queries
3. Polypharmacy medication reviews and SMRs
4. Face to Face medication reviews
5. Help to utilise skill mix
6. Improve patient outcomes
7. Ensure better access to healthcare
8. Support practice staff with prescription and medication queries
9. Medicine reconciliation and review post-discharge
10. Conduct medical related audits and work on service improvement with the practice
11. Provide expert advice on medications to other clinicians and patients
12. Deliver clinical leadership on medicines optimisation and quality improvement
13. Ensure appropriate and efficient use of medication
14. Educate patients on medication and disease prevention
15. An understanding of QOF, CQC and local incentive scheme workstream
16. Create and follow clinical pharmacy programme according to practice policy

Job responsibilities
17. A minimum of 12 months experience of working within general practice/primary care
18. A relevant degree/qualification to show you are registered Clinical Pharmacist with GPHC registration
19. Independent prescribing qualification
20. Experience with EMIS or System 1
21. Desirable to already be on CPPE Primary Care pathway or will be willing to partake, if you have extensive experience working as a Clinical Pharmacist an exemption route can be sourced.
22. Able to commute to onsite location / live within close proximity of practice
23. Excellent communication and interpersonal skills
