Job summary
An exciting opportunity has arisen for a motivated, dedicated and innovative Physiotherapist to join our workforce. We are a small, multidisciplinary team whose primary aim to prevent admission to hospital of patients within Huntingdonshire. We work within an acute hospital's Emergency Department (ED) and Acute Assessment Unit (AAU), receiving referrals from consultants requesting assessments of patients suitable for discharge.
You will be expected to support the wider team within the ED and AAU, working with acute MDT colleagues to assess, plan and facilitate a safe discharge, referring onwards and signposting as indicated based on the patient presentation.
You should have significant post registration experience including experience working at a Band 5 level (or above) in both acute and community settings, covering a wide variety of areas, including the ability to read x-rays and scans.
You will need to be able to demonstrate specialist knowledge in the assessment and treatment of patients; an understanding of the National Guidelines / Standards, as well as evidence of post graduate training.
Highly developed communication skills, flexibility, good organisational skills and time management skills are all essential criteria. Basic IT skills are also required
Main duties of the job
Identify patients within Emergency Department, who may meet the admission criteria for CPFT services.
To assess the identified, or patients referred by consultants in Emergency Department.
To document assessments and findings on SystmOne, Etrac, and Symphony.
To liaise with consultants and senior nurses in the department.
To liaise with the social worker regarding patients who do not meet Intermediate Care admission criteria or who have obvious need for social care support.
To support students, learners, and other team members.
