Job summary Do you have clinical background skills and a desire to enhance the patient discharge process. You will work as part of the Same Day Discharge Centre ensuring a seamless transfer of patients from wards to the Centre. Whilst in the Same Day Discharge Centre you will ensure that patients receive ongoing care and support whilst finalising safe and effective discharge arrangements. We are looking for an enthusiastic motivated team member who will actively pull patients into the service working in collaboration with the Trust Capacity Team ensuring patient flow. Main duties of the job To ensure that patients are safely discharged home or to another care setting as per their plan of care whilst seamlessly continuing their care up until the point of leaving hospital. To deliver all aspects of care up until discharge which will include medicines management, direct patient care, community referrals and last dose IV antibiotics if clinically appropriate. To actively pull patients identified as confirmed discharges into the Centre to support timely and effective flow across the Trust.
