Job overview
Continuous expansion in our Renal Home Therapies (RHT) programmes requires appropriate & skilled workforce. We aim to provide the best care and keep patients safe at home. We are looking for a dynamic, enthusiastic, hardworking and passionate renal nurse. This role will enhance your knowledge, skills and experience both in Peritoneal Dialysis (PD) & Home Haemodialysis (HHD) and gain management experience. You will have opportunities to work autonomously and collaboratively with the Multi-Disciplinary Team.
A skilled renal nurse with substantive renal experience (passionate for teaching and excellent communication skills), in possession of a relevant specialist and recognised renal course and able to travel/make use of public transport for home/community visits are the essentials.
We currently have 3 sites delivering HHD training, including the main hub for the wider RHT located at Hammersmith Hospital (HH). PD Training is carried out externally with the vision to bring it on site in the future.
A full UK driving licence as well as an experienced, competent and skilled in handling home dialysis machines: NxStage in HHD & Claria APD in PD is desirable.
Main duties of the job
* To support the home therapies patient program, providing high quality, effective, individualized care through the assessment of patient care needs to develop, implement and evaluate them with supervision.
* To prioritize a program of education and training that is individualized to patients’ needs that ensures consistency, thoroughness and safety. To support the patient, independently planning safe transfer home in a timely manner.
* Monitor and maintain high standards of nursing care and take appropriate action for continuous improvement.
* To respond to patients, relatives and carers’ concerns as they arise and take remedial action as required.

Person specification
Education/ Qualifications
* Registered Nurse
* Evidence of continuing professional development demonstrating advanced assessment skills
* Teaching Qualification (ENB 998/Slice or equivalent experience)
Experience
* Renal experience (> 6 months)
* Extensive experience both in PD and in HHD - acute and chronic.
* Experienced, competent, knowledgeable and skilled in handling dialysis machines, e.g., Claria APD, NxStage HHD machine, etc.
* Previous work experience in PD or Home HD
* Experience of being a Team Leader / Deputy In charge
Skills/Knowledge/ Abilities
* Organization and management skills.
* Excellent teaching skills.
* IT skills including use of email and office packages.
* Competency in medication administration including IV drugs.
* Knowledge of budget management.
* Knowledge of Health & Safety management.
