Job overview
The Care Coordination Team are motivated and passionate about providing high quality, evidence-based care to patients. We need exceptional individuals with a passion for delivering high quality care with a focus on patient safety.
We are now looking for a Specialist Nurse Practitioner to join the Care Coordination Team. We support emergency services in directing patients to the right place of care and supporting our acute colleagues to discharge patients in an appropriate timeframe – ensuring continuity of care is delivered at home as required.
If you think you have what it takes or you are looking for a change and want to work more autonomously in a challenging environment, with opportunities for further career development then the community could be for you. The staff work closely with members of the wider multi-disciplinary teams.
You must hold a full UK driving license and have access to a car for work purposes.
Main duties of the job
The post holder will be required to have experience in triage and care of the acutely ill adult, demonstrating competency in both rapid and holistic triage to ensure patients are signposted and referred to the correct service to meet their needs. You must be confident in completing telephone triage and utilizing web-based applications to complete video triage and remotely monitor patients. You must be IT literate and be able to work at a fast pace to meet demand.
The post holder will be required to complete face-to-face assessments for patients in their homes or an acute setting when required and therefore must be competent in elements of care delivery as required of a Specialist Nurse Practitioner. The post holder must have health assessment and prescribing qualifications.
