Main duties of the job
The post holder is responsible for the delivery of day-to-day practice nursing services to the practice population.
Supported by the clinical team within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities both in Practice and in the community. They will work collaboratively with the general practice team to meet the needs of the patients and to refer into complementary services as required; supporting the delivery of policy and procedures and providing nurse leadership as required.

Job responsibilities
The focus of the role is both the delivery of evidence-based practice for Patients presenting with a long term condition, and the provision of preventative health care to the Practice population.
As an autonomous Practitioner, the Practice Nurse is responsible for the care delivered to the population of Bury, demonstrating critical thinking and skills in clinical decision-making in the management of Patients. They will work collaboratively with the General Practitioner and other Practice Nurses to support the delivery of a healthier Bury through listening and responding to Patients needs.

Essential
1. Registered general Nurse.
2. Current NMC Registration
3. GCSE Grade A to C in English and Maths
Desirable
4. Community Nursing Specialist Qualification or Equivalent.
5. Relevant Nursing Degree.
Experience

6. Clinical Supervision and Training Experience.
7. Experience of working within Primary Care.
8. Experience of quality initiatives such as clinical benchmarking.
9. Experience of management of patients with long term conditions.
10. ECGs Venepuncture New Patient and other medicals Chaperoning support Requesting pathology tests and processing the results, advising patients accordingly Immunisations (routine, childhood and travel) Diabetes, Hypertension, Asthma, Spirometry, CHD Womens health (cervical cytology and contraception etc.)
11. Understand importance of evidence based practice.
12. Broad knowledge of clinical governance Ability to record accurate clinical notes Knowledge of health promotion strategies
