Young person requires support with enjoying life
Preferred days/times:
6 hours a week, time to be decided, dependent on when attending college
Hours to be split over two days
Tasks/duties:
Support with some household tasks. The person needs support with keeping on track with tasks as they will forget what they are doing. Support with setting up a household task rota
Support required with accessing the community. The client has no concept of money and cannot go out into the community alone.
Support with coping strategies, route planning, stranger danger, road safety awareness, and how to ask for help
Support with learning how to access resources and voluntary experiences aimed at the client’s long-term goal of gaining employment
Client main diagnosis:
Foetal Alcohol Syndrome – the client’s senses, motor fine skills and balance are affected. They can be prone to trips and falls. They always have a teddy with them.
They have traits of autism, cannot tolerate loud noises, being touched, or food touching.
Learning difficulties. The client uses audio technology and requires instructions to be in small manageable steps. They cannot read and write and have tremors in their hands.
Communication:
The client has no communication barriers
PA to have their own vehicle and a full, clean, UK driving licence
Client enjoys:
collecting Barbies and soft toys, and they love build-a-bear. The client carries a soft toy with them all of the time
