Overview
The Annual Wellness Visit nurse works in collaboration with the primary care team, all members of the care delivery and population health team. The AWV Nurse is responsible for identification of patients appropriate for Annual Wellness Visits and independently completing the visit. Their objective is to complete the Annual Wellness Visit in its entirety in a timely fashion; discuss health prevention; educate on related topics; refer to care coordination and be knowledgeable of community resources to support prevention of avoidable hospital readmissions and unnecessary emergency room visits.
Responsibilities
* Follows AWV protocols and standing orders including completion of preventive services.
* Develops and shares knowledge of AWV eligibility and documentation requirements.
* Develops knowledge of Medicare preventative services and clinical guideline recommendations for those services.
* Addresses open quality gaps with patients and works collaboratively with patients towards closure of those gaps.
* Appropriately documents within the electronic record to satisfy closed quality gaps.
* Works collaboratively with clinical teams to accomplish AWV in an efficient manner for patient and team. This will include scheduling of the patients and filling open appointments to maintain AWV goals.
* Identify the need for patient education. Advise patients on their treatment plan.
* Refers appropriate patients to care coordination for improved outcomes.
* Maintain treatment records/files for each patient.
* Completes patient encounter form at the conclusion of each service performed.
* Maintain patient confidentiality; comply with HIPAA and compliance guidelines.
* Reports outstanding findings to provider immediately.
* All other duties as assigned within the scope and range of job responsibilities.
Qualifications
* Required: Two (2) or more years clinical nursing experience with 1 year in an outpatient setting. Experience with Medicare Annual Wellness Visits as well as EHR experience.

* Ability to work independently and exercise clinical judgment in interactions with Physicians, payors, patients, and their families.
* Strong organizational and time management skills, as evidenced by a capacity to prioritize multiple tasks and role components.
* Aware of scope of practice boundaries, comfortable seeking direction and assistance from appropriate resources.
* Communication: Ability to communicate effectively verbally and in writing. Excellent negotiation skills.
* Computer Skills: Proficient ability to use a computer and electronic medical record.
* Confidentiality: Maintain patient, team member and employer confidentiality. Comply with all HIPAA regulations.
* Customer Service Oriented: Friendly, cheerful and helpful to patients and others.
