General Practitioner for Primary Care Network - Fareham & Portchester PCN
Job summary
We are seeking a dedicated General Practitioner to join our dynamic team. The ideal candidate will have a passion for delivering health care and a commitment to continuous improvement. You will be responsible for providing comprehensive Primary Care Services to patients of all ages, managing both acute and chronic conditions and promoting overall health and wellness.
This role will be a split post between the Home Visiting Service (HVS) and the four member practices of F&P PCN, with the division of time based on demand. It will involve providing acute and proactive care to patients in their own homes or in residential and nursing care as part of the HVS, as well as delivering same-day care within the practices across all four sites.
This will be for a fixed term post for 1 year with a likely extension dependent on Government announcement.
Successful candidates will be within two years of their certificate of completion issued by the GMC. Start and finish times are flexible, but the post consists of around one session per day.

Fareham & Portchester PCN (FPPCN) comprises 4 practices: Fareham Centre Practice, Gudgeheath Lane Surgery, Westlands Medical Centre, and Portchester Practice. We have a patient population of approximately 46,500 with a diverse workforce of Paramedics, Nurses, General Practice Assistants, Care Co-ordinators, FCP Physios, Social Prescribers, and a Clinical Pharmacy team. Main duties of the job
1. Ordering and interpreting diagnostic tests.
2. Collaborating with specialists and other healthcare professionals to ensure comprehensive patient care.
3. Maintaining accurate and up-to-date patient records.
4. Educating patients on health management and disease prevention.
Person Specification
Qualifications
* Medical degree from an accredited institution.
* Valid medical licence to practice as a GP in the Hampshire and Isle-of-Wight area.
* You must be within two years of the date of your certificate of completion of training (CCT) issued by the General Medical Council.
* Strong clinical skills and knowledge of Primary Care.
* Good working knowledge of the EMIS clinical system.
* Excellent communication and interpersonal skills.
* Ability to work effectively in a team environment as well as alone.
* Ability to manage under pressure.
* Commitment to providing compassionate and patient-centred care.
