Requirements:
* Experience of working with adults with long term conditions.
* Experienced Qualified Social Worker Required.
* Experience of Court of Protection work.
* Experience of safeguarding.
* Experience of working with adults and self neglect.
What we are looking for in a Social Worker:
* Social Work England registration
* Eligible to work in the UK
* Hold a full UK licence
* Post-qualified experience
* Degree level or equivalent in Social Work
Your duties will include:
* Admission avoidance and prevention: Ensuring people are safe.
* Person-centred discharge planning: Using a strengths-based approach.
* Multi-disciplinary working: Collaborating with Occupational Therapists, Physiotherapists, and other medical professionals.
* Advocacy: Ensuring people’s voices, including carers and unpaid carers, are heard.
* Encouraging independence: Helping people grow their confidence and make the most of available support.
* Management of complex cases: Making evidence-based decisions in a fast-paced environment.
