Who You Are An experienced and qualified Social Worker with a strong understanding of the continuing health care process, procedures, and relevant legislation, including Decision Support Tools and the London Health Needs Tool. You possess excellent assessment and reporting skills, and are capable of developing detailed support plans. You are sensitive in communicating with family members of deceased service users and are adaptable in working across multiple sites and using various databases for information management. What the Job Involves This role requires collaborating with health colleagues on retrospective continuing health care cases. You will conduct comprehensive social care assessments and produce professional reports and support plans as required. The position may also involve interacting with the family members of deceased service users in a respectful and empathetic manner. Additionally, you will navigate multiple locations and manage documentation across different databases effectively.
