Main Duties of the Job
1. Conduct thorough assessments for NHS Continuing Healthcare for Adults to guide care decisions.
2. Coordinate and monitor complex care packages and NHS-funded placements, ensuring patient safety at all times.
3. Strengthen links between primary care, social services, and other stakeholders, working closely with colleagues across trust, community, and private sectors.
4. Provide ongoing case management to ensure high-quality care and optimal patient outcomes.
5. Maintain accurate and up-to-date records, participate in the CHC panels as needed, and ensure compliance with regulatory standards.
Required Skills and Qualifications:
1. Current RGN registration.
2. Experience in case management and patient assessment, with an understanding of the CHC framework.
3. Exceptional communication and liaison skills, capable of working across inter-agency boundaries.
4. Excellent time management and organisational skills to manage multiple responsibilities efficiently.

Job Responsibilities
1. Provide a high-quality resource of professional advice, guidance and expertise to members of the Individual Care Team, ICB and wider health and social care economy.
2. Autonomously manage a case load of individuals with highly complex health needs providing significant clinical guidance to providers.
3. Ensure that all assessments are holistic in nature and utilise The National Framework for NHS Continuing Health Care.
4. Provide complex and sensitive information to patients, relatives/representatives and often work in potentially hostile environments; relaying difficult decisions regarding care provision and managing patient and family expectations.
5. Provide continuity of care throughout the period of care and support, encompassing close working with health and social care professionals and care providers.
6. Attend all meetings related to the patient which may impact on care provision and costs to meet identified needs.
7. Liaise with multiple professionals involved in patient care to ensure a comprehensive understanding of all identified needs.
8. Present comprehensive written reports to ICB Eligibility Panel following any reviews completed once patients are in receipt of agreed care provision.
9. Responsible for negotiating care package costs and monitoring packages of fully funded care which are in excess of £1000 per week.
10. Identify and process any safeguarding and quality of care issues and take appropriate action.
11. Ensure Mental Capacity Act is considered and relevant processes are followed.
12. Develop and maintain effective working relationships with clinicians, general practitioners and providers.
13. Provide professional advice, guidance and expertise to the ICB and partner agencies.
14. Liaise with a variety of providers and arrange assessment of patients.
15. Maintain a formal record of all cases and ensure that all relevant clinical information is available.
16. Support the Head of CHC in working with partner agencies.
17. Call and chair multi-disciplinary meetings to ensure continuity of care across agencies.
18. Deputise for the Lead Nurse as required.
19. Provide data to monitor care delivery and identify how performance of providers can be improved.
20. Monitor budget and inform the Head of Service of any identified budget pressures.
21. Develop policies and procedures to support the delivery of NHS Funded Care.
22. Contribute to and deliver training on CHC and FNC to a variety of audiences.
23. Provide professional support and mentoring to the nurse assessors within the team.
24. Work within the NMC Code/Professional Code and meet requirements for revalidation.
25. Undertake and comply with all mandatory training.

Skills and Knowledge
Essential
1. High level of working knowledge of the relevant legislation and theories underpinning the provision of services.
2. Demonstrable experience and knowledge of current key policies and priorities within the NHS.
3. Clear knowledge and understanding of service provision available locally, regionally and nationally.
4. Good working knowledge of adult safeguarding.
5. Awareness of Data Protection and confidentiality issues.
6. Working knowledge of Microsoft Office.
7. Effective interpersonal and negotiation skills.
8. Excellent written and verbal communication.
9. Excellent time management skills.

1. Relevant degree/professional qualification in Nursing.
