Job summary
Be an integral member of the integrated community team and, underthe direction of the qualified clinician, provide care and rehabilitation forpatients in their own homes or clinics. This care will be based on thequalified professionals assessed need for each patient and will be carried outto the agreed standards taking account of Trust policies, procedures andguidelines

Main duties of the job
The role will require:
1. The ability to communicate effectively withothers at all levels
2. Working flexibly to provide the servicedelivery need across the local area (the service operates from 7:30am 6:30pm,Monday to Sunday including bank holidays)
3. An enthusiastic, motivated and innovativethinker to become part of a primary care facing community
4. The ability to work autonomously and workingwithin a multi-disciplinary team
5. The ability to problem solve and manage timeeffectively
6. Experience of working in a fast paced andvaried environment
7. The ability to travel to deliver care servicesyou will have the use of a Trust pool car when allocated on a roster basis.

Essential
14. Level 3 qualification ( NVQ 3, A-level) in Care Or Level 2 qualification ( NVQ2, GCSE Grade A-C) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post
15. Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust
16. I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
