Job summary Are you a highly motivated Registered Nurse with a minimum of 2 years post registration experience of working with patients diagnosed with diabetes that would enjoy working in Primary Care, as a Diabetes Specialist Nurse (DSN). We have a position for a Band 6 DSN to work in the Integrated Diabetes team, delivering excellent diabetes care and support to patients across Aneurin Bevan University Health Board. This role is key to enhance skills and knowledge within primary care services to support people living with diabetes. Main duties of the job The role is crucial in the optimisation of lifestyle, education & medicine therapies in line with NICE & local guidelines and supporting people with diabetes in primary and community care settings in the initiation and management of therapies including injectable agents. Reporting to PCDSN team leader, the post holder will be required to establish relationships and specialist clinical leadership to support the delivery of an Integrated Diabetes Specialist Care Team. There is an expectation that the DSN will work closely with a range of primary care practitioners, secondary care colleagues and care homes, across all care interfaces and there is an expectation for post holders to travel independently across the geographical area of Gwent to undertake the role. The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
