The following are the core responsibilities of the Clinical Pharmacist. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
1. Practice as an independent prescriber working within scope of practice (when qualified).
2. Provide medication review services with patients in each PCN by telephone calls, clinical records reviews, face to face reviews, or during domiciliary visits.
3. Reconcile medicines following patients' discharge from hospital or clinic appointments.
4. Review all safety alerts ensuring the required actions are taken.
5. Carry out targeted medication reviews for high-risk patients (e.g., by using eclipse).
6. Review and implement safe prescribing systems for vulnerable patients, improving compliance.
7. Provide subject matter expertise on medication monitoring, implementing and embedding a system.
8. Undertake practice/PCN and Federation level audits.
