Main duties of the job
Provideexpertise in clinical medicines review and address public health and socialneeds of patients in Hillsborough Primary Care Network.
The post holder will work as part of amulti-disciplinary team in a patient facing role. The post holder will takeresponsibility for areas of chronic disease management within the practice andundertake clinical medication reviews to proactively manage patients withcomplex polypharmacy.
The post holder will provide primarysupport to general practice staff with regards to prescription and medicationqueries. They will help support the repeat prescriptions system, deal withacute prescription requests, and medicines reconciliation on transfer of careand systems for safer prescribing, providing expertise in clinical medicinesadvice while addressing both public and social care needs of patient in the GP practice(s).
The post holder will provide clinicalleadership on medicines optimisation and quality improvement and manage someaspects of the quality and outcomes framework and enhanced services.
The post holder will ensure that thepractice integrates with community and hospital pharmacy to help utilise skillmix, improve patient outcomes, ensure better access to healthcare and helpmanage workload. The role is pivotal to improving the quality of care andoperational efficiencies so requires motivation and passion to deliverexcellent service within general practice.

Primary Duties and Areas of Responsibility
See(where appropriate) patients with single or multiple medical problems wheremedicine optimisation is required (e.g. COPD, asthma). Review the ongoingneed for each medicine, a review of monitoring needs and an opportunity to supportpatients with their medicines taking ensuring they get the best use of theirmedicines (i.e. medicines optimisation). Make appropriate recommendations toGPs for medicine improvement
Undertakeclinical medication reviews with patients and produce recommendations for thenurses and/or GP on prescribing and monitoring
Undertakeclinical medication reviews with patients and produce recommendations for thecommunity nurses or GPs on prescribing and monitoring. Work with care homestaff to improve safety of medicines ordering and administration

Medicine information topractice staff and patients
Answersrelevant medicine related enquiries from GPs, other practice staff, otherhealthcare teams (e.g. community pharmacy) and patients with queries aboutmedicines. Suggesting and recommending solutions. Providing follow up forpatients to monitor the effect of any changes
Unplanned hospitaladmissions
Reviewthe use of medicines most commonly associated with unplanned hospitaladmissions and re-admissions through audit and individual patient reviews.Put in place changes to reduce the prescribing of these medicines tohigh-risk patient groups
Management of medicines at discharge from hospital
Toreconcile medicines following discharge from hospitals, intermediate care andinto care homes, including identifying and rectifying unexplained changes andworking with patients and community pharmacists to ensure patients receivethe medicines they need post discharge. Set up and manage systems to ensurecontinuity of medicines supply to high-risk groups of patients (e.g. thosewith medicine compliance aids or those in care homes)
Signposting
Ensurethat patients are referred to the appropriate healthcare professional for theappropriate level of care within an appropriate period of time e.g. pathologyresults, common/minor ailments, acute conditions, long term condition reviewsetc.
Repeat prescribing
Produceand implement a practice repeat prescribing policy. Manage the repeatprescribing reauthorisation process by reviewing patient requests for repeatprescriptions and reviewing medicines reaching review dates and flagging upthose needing a review. Ensure patients have appropriate monitoring tests inplace when required
Contributepharmaceutical advice for the development and implementation of new servicesthat have medicinal components (e.g. advice on treatment pathways and patientinformation leaflets)
Information management
Analyse,interpret and present medicines data to highlight issues and risks to supportdecision making
Undertakeclinical audits of prescribing in areas directed by the GPs, feedback theresults and implement changes in conjunction with the practice team
Implementchanges to medicines that result from MHRA alerts, product withdrawal and otherlocal and national guidance
Implementation of localand national guidelines and formulary recommendations
Monitorpractice prescribing against the local health economys RAG list and makerecommendations to GPs for medicines that should be prescribed by hospitaldoctors (red drugs) or subject to shared care (amber drugs). Assist practicesin seeing and maintaining a practice formulary that is hosted on the practicescomputer system. Auditing practices compliance against NICE technologyassessment guidance. Provide newsletters or bulletins on importantprescribing messages
Education and Training
Provideeducation and training to primary healthcare team on therapeutics andmedicines optimisation
Care Quality Commission
Workwith the general practice team to ensure the practice is compliant with CQCstandards where medicines are involved
Public health
Tosupport public health campaigns. To provide specialist knowledge on allpublic health programmes available to the general public

* Degree in Pharmacy, and registration with the General Pharmaceutical Council.
* Member of the Royal Pharmaceutical Society (RPS).
* Member of or working towards (through foundation) RPS Faculty membership Hold or be working towards a prescribing qualification.
