As part of the innovative and ongoing development of our Stroke Services at Liverpool University Hospital NHS Trust we are looking for dedicated and dynamic applicants to join our highly motivated and proactive team. Excellent and advanced clinical knowledge of Stroke care is essential with the determination and drive to make a difference. The successful candidate will need to have excellent interpersonal skills and good verbal and written communication skills. You will be familiar with national guidelines, pathways and treatments.
You will be highly motivated and demonstrate the ability to work independently, autonomously and safely within your clinical competency. As a positive role model you will lead by example and uphold the high standards of care and efficiency delivered by the service.
We are passionate about this role, and in ensuring you are supported and developed continually, with appropriate supervision from your medical colleagues, whilst ensuring you play a key role within the multidisciplinary team.
1. To improve the experience and outcomes of care for patients by taking the lead in investigations and treatmentsprescribed.
2. To provide a consistent and personalised service by ensuring that the patients receive the best care, in the best place from the best person orteam.
3. To provide social, emotional and psychological support for patientsfrom referral and throughout theirtreatment.
4. To deliver expert specialist evidence based care including: development, assessment, planning, implementing, evaluating programmes ofevidence based nursing care for a definedcaseload.
5. To make appropriate referral to consultants/other specialist/members ofthe multidisciplinary teaminprimary, secondary or tertiary caresettings.
To be aware of risk management issues both locally and nationallyand develop strategies to address identifiedissues.

Key responsibilities
Clinical.
1. To maintain personal accountability for professional and ethical actionsand ensure compliance withNMCcode of professionalconduct.
2. To maintain and develop a specialist nursing service for patients within the caseload, ensuring that the treatment and follow up of this group of patientsis deliveredina timelymanner.
3. To provide and maintain a diagnostic service for the Trust. Ensure nurse-led clinics provide for both diagnoses and reporting of subsequent results for identifiedpatients.
4. To provide teaching as an in-house measure to all medical and nursingstaff, not only within the Directorate but also from otherservices.
5. To provide a nurse-led clinic service topatients.
6. To plan and administer treatment to patients as appropriate andensure handover of this treatment to community services asnecessary..
7. To provide expert nursing care, advice and support for patients andtheir relatives/carers and ensure that advice and point of contact is available throughout the patient’s journey.
8. To refer on to other agencies asappropriate.
9. To operate as clinical specialist and expert practitioner in issues relating to the identified area of nursing.Todirect and delegate nursing care based on accurate assessment andevaluation.
10. To take consent for investigative procedures, manipulate equipmentand perform advanced clinical skills.
Management and Quality.
1. Act as a change agent for the development of theservice.
2. Assisting in the audit and data collection required for developingevidence basednursing.
3. To provide verbal and written reports to appropriate managers andclinical leads.
4. To monitor the effectiveness of the service by continuousaudit.
5. To ensure that equipment is maintained and serviced asrequired.
6. To develop protocols, documentation, standards and guidelines toenable best practice for the care of patients with urologicalconditions.
7. To be involvedincomplaints handling according to the Trust’s Complaints Procedure.
8. To establish a clear focus, vision and direction for the nursing service,within theteam/directorate.
9. To ensure effective and efficient use of financial resources, contributing to increasing the efficiency and controlling cost within the departmentalbudget. Making recommendations regarding supplies and purchasing ofequipment.
Communication and Liaison
1. To promote effective communication within the hospital and to the patient and/or theirrelatives.
2. Act as a point of liaison between all hospital departments involved inthe patient’sjourney.
3. Act as a source of specialist information for nurses and other membersof the multidisciplinaryteam.
4. Liaise with other departments/ Trusts regarding services providedand current practises to develop best evidence basedpractice.
5. To be skilled to impart distressing information with respect andempathy.
6. To ensure that all patient records are maintainedcontemporaneously.
*The post holder shall as necessary provide cover for and undertake duties of absentcolleagues.
* The post holder shall follow all the policies and procedures of the organisation.
Education and training development
1. Maintain own professional knowledge base and skills, meet PREP requirements, and continually updateknowledge.
2. To provide education to members of the multidisciplines, relating to thearea of specialistknowledge.
3. To promote and give presentations about the role of the specialist nursein service.
4. To act as an educator and assessor to promote and maintain skills and knowledge to relevant medical and nursing staff, including specialistregistrars and senior houseofficers.
5. To be actively involved in health promotioninline with Governmenttargets.
