If you are looking to develop your NHS experience and provide social care with the support of working within a multidisciplinary team- then please do apply The Outreach team aims to support and engage children, young people and families with complex needs by means of individualised home and community treatment packages. What you can add: Reduce children and young people’s vulnerability to mental health crisis Maximise children and young people’s resilience Enable children and young people with high levels of mental health need to be supported in their own homes or placements Offer choice to children, young people and their families and carers Avoid inpatient admissions Reduce inpatient days Offer advice, support and supervision to systems supporting young people, such as addiction, adoption, Children’s Care and youth offending services What you’ll need: Degree in Social Work or equivalent qualification Post qualification experience in a mental health setting, working with young people or adults with mental illness. Ability to travel between sites and to regional meetings To have ability to manage case load and act as care coordinator for clients on case list In addition to experience, all Qualified roles will be required to provide evidence of: Relevant Qualifications Registration with the relevant Registration Body if applicable.
