Clinical Work in partnership with service users and families to develop, review and implement programmes of care, delivering advice to support health promotion, improved quality of life and prevent illness Utilise specialist skills during assessment and interpretation of acute and chronic conditions, escalating when appropriate Communicate effectively with the wider team and patients, including sharing ideas, information and feedback Promote health through empowering patients and their carers by offering appropriate education and advice to enable patients to make informed decisions about their care Demonstrate an understanding of Haemoglobinopathies, the implications of living with a chronic illness and its impact on individuals, families and the wider community Support and lead nurse led clinics, telephone and face to face, interpreting results, taking appropriate action and feeding back in MDT meetings and reviews Support the transition of adolescents to the adult services within the trust To be responsible for the assessment, escalation and management of risk in accordance with the trusts risk management policy Assist in the organisation of treatment plans ensuring patients are treated within a timely manner Managerial/Leadership Contribute to the development of Haemoglobinopathy protocols and care plans Work collaboratively with the Haematology team to establish clear roles and responsibilities for managing of the MDT, audits and ward rounds Keep informed of national and Trust policies, especially those affecting service delivery and patient care Promote a culture that supports patient centred care and patient empowerment Act as role model demonstrating high standards of care and providing clinical leadership to others Actively participate in MDTs, acting as the patient advocate and representing nursing and patient views Education Provide education to service users and their families regarding their disease Provide teaching and training on Haemoglobinopathies to the wider team and trust, demonstrating expert knowledge and advice in own specialist area Demonstrate knowledge on Haemoglobinopathies and be willing to learn or expand expertise in this area Take personal responsibility for life-long learning and personal development, engaging in learning and development opportunities To be a non-medical prescriber and prescriber of blood products, or be willing to work towards these competencies. Service Improvement Support the ongoing development of the apheresis project, working in collaboration with the multidisciplinary team Work in partnership with service users and families to develop, review and implement evidence based individualized care plans Attend patient forums where appropriate in order to enhance service improvement Develop new skills in response to emerging knowledge and techniques Promote patient and public experience feedback through local initiatives to drive innovation and change Maintain accurate data relating to the clinical service ensuring strict confidentiality Research & Audit Influences and participates in development of policy and clinical guidelines Work closely with colleagues to develop evidence based practice within this area of nursing Participate in relevant local and national audit for specialist group
