A fantastic opportunity has arisen for a Diabetes Specialist Nurse to join our Huntingdon Team working full time hours per week. If you have compassion, empathy with the ability to work autonomously, this is the opportunity for you.
You will work as part of the multidisciplinary team with the aim of preventing admission and providing ongoing care for patients with Diabetes Disease within the community.
You will also be responsible for managing a caseload of patients with complex Diabetes needs, using evidence based practice and patient centred goals to assess plan implement and evaluate inventions
The role includes promoting better understanding of Diabetes for patients, their families and carers and other agencies involved with the patient through education, support and counselling.
Main duties of the job
1. To act as key worker as appropriate, co-ordinating multidisciplinary care
2. To provide expert support, advice and education to raise awareness of the needs of patients with Diabetes
3. To undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues
4. To work closely with both primary and secondary care to ensure a safe and effective patient pathway is in place for all diabetes patients under the care of the community diabetes team
