Cambridge City 4 Primary Care Network Ltd. offers an exciting opportunity to join our established team of 4 Clinical Pharmacists and 5 Technicians based around our 6 network GP practices in central Cambridge. As a Primary Care Pharmacy Technician you will be working across the Cambridge City 4 Primary Care Network to support the delivery and development of our Pharmacy services, carrying out clinical medication reviews, high risk drug monitoring, supporting vaccination clinics, processing discharge medication and medication management. You will be working in partnership with the patients GP practice, community pharmacists, nurses, care homes, secondary care as well as the extended PCN personalised care and physiotherapy teams.

Main duties of the job
To support the management and development of the prescribingsystems to ensure safe, effective and quality prescribing within the practices.
1. To work closely with the lead GP partners for prescribing onindividual prescribing projects or audits.
2. To develop strong working relationships with communitypharmacies.
3. To act as lead contact for any complex prescribing queries fromstaff.
4. Support the practice drug monitoring protocols, as per NICE guidelines.
5. Demonstrate knowledge and understanding of the local formulary guidelines and use of digital resources eclipse.
6. Communicating with patients/carers and members of thehealthcare team including GP's, nurses and receptionists.
7. Liaising with local pharmacies and hospitals regardingdischarge queries, and helping to ensure optimum therapy and to ensure asmooth transition from Secondary to Primary Care.
8. Answer ad hoc medication drug queries from GPs, nurses andother healthcare professionals within the post holders expertise.
9. support clinicaladministration, projects and audits to improve services and initiatives toenhance safety, quality and cost effectiveness.
10. to work autonomously and be capable of prioritisingwork whilst maintaining accurate and timely records.
11. Partake in PCN pharmacy team meetings with a positive and encouraging attitude towards team work, PCN prospects, problem solving and future development .
12. To facilitate and participate in multidisciplinary meetingsand educational sessions related to prescribing topics.

Job responsibilities
Main Purpose:
13. To support the management and development of the prescribingsystems to ensure safe, effective and quality prescribing within the practice.

17. Dealing with medication queries including acute medication requests,patient medication and prescription queries. This may involve removing unwanteditems, checking review dates, ensuring necessary blood tests and monitoring hasbeen carried out, prescription synchronization etc.
18. Communicating with patients/carers and members of the healthcare teamincluding doctors, nurses and receptionists.
19. Liaising with local pharmacies and hospitals regarding prescriptionqueries, and helping to ensure optimum therapy and to ensure a smoothtransition from Secondary to Primary Care.
20. Answer ad hoc medication drug queries from GPs, nurses and otherhealthcare professionals within the post holders expertise.
21. Updating of medication received from secondary care on the patientmedical record, where appropriate.
22. To problem solve issues related to prescribing.
23. To support the lead GP for prescribing with clinical administration,projects and audits to improve services and initiatives to enhance safety,quality and cost effectiveness of prescribing ( Prescribing Quality Schemeas assigned by CCG Medicines Management Team, MHRA updates etc.)
24. To workas an autonomously and be capable of prioritising workwhilst maintaining accurate and timely records.
Other Duties:
25. To facilitate and participate in multidisciplinary meetings andeducational sessions related to prescribing topics.
26. To keep professionally up-to-date at all times and to meet the GeneralPharmaceutical Council standards for continuing professional development so asto maintain professional registration.
27. To ensure that all information is recorded accurately using computersystems and paper if required.
28. To ensure maintenance of accurate patient records and ensure allrelevant information is documented in the patients medical record in a timelymanner.
Key working relationships:

The post holder will be required to maintain constructive relationshipswith a broad range of internal and external stakeholders and communicate withall grades of healthcare staff including all practice staff, secondary healthcare staff, community health care professionals and community pharmacists.

55. * Professional registration with GPhC
56. * BTEC/NVQ level 3 or equivalent in pharmaceutical sciences
57. * Experience of working as a qualified, registered Pharmacy Technician in primary care/secondary care/community care
58. * Knowledge and understanding of pharmacy law, ethics and current legislation
59. * Good clinical pharmacy knowledge including terminology
60. * Sound computer skills, proficient with MS Office
