1. Lead and/or participate in the Team Around the Family (TAF) process, adhering to Cheshire West and Chester TAF Guidance, to ensure the production of an holistic assessment; based on the families’ identified needs, a SMART plan and a multi-agency coordinated outcomes focused approach.
2. Conduct high quality direct work, including evidenced based interventions, with parents/carers and children to promote resilience, affect positive change in behaviours, development skills and their ability to resolve multiple and complex problems.
3. Establish and enable effective working relationships with statutory and voluntary agencies to ensure a high standard of service is provided to families.
4. Be an advocate for families, assisting them in reducing barriers and blockages to support services enabling families to achieve sustainable outcomes.
5. Assertively work in partnership with the family; particularly those who have been identified as being challenging to existing service providers, and other agencies to prevent the need to implement foreseeable enforceable sanctions such as loss of benefits, eviction, loss of access to children, serving of anti-social behaviour orders.
6. Provide professional challenge to partners, including use of the LSCB Pan Cheshire Escalation policy where appropriate.
7. Current awareness of and compliance with statutory requirements, CWaC policies, inter-agency protocols, equality and diversity and other regulations and procedures in order to ensure statutory and Departmental requirements are met, including:
8. Troubled Families programme, Department of Work and Pensions programme and Ofsted.
9. Record all case work as per CWaC Recording Policy. This includes maintaining accurate, good quality records that evidence the child/young person’s views and their lived experience
10. Prepare for and participate in supervisory, appraisal and training processes to ensure that you are a reflective practitioner whose practice is relevant, outcome focused, inquisitive and of a high quality.
