The post holder will provide day-to-day clinical nursing/therapy expertise and co-ordination to a multi-disciplinary integrated discharge hub based at the Newham Hospital working in partnership to facilitate timely discharge of patients from hospitals. The team consists of nurses, therapists, social workers, and admins.
The post holder will:
1. Undertake 'in reach' triage assessments for patients on the acute Wards at Newham Hospital to ensure rapid discharge. Undertake generic IDH clinical assessment and coordination tasks and functions.
2. Coordinate and chair meetings face-to-face or virtually held at different times on weekdays and weekends with system partners and discharge hubs.
3. Liaise with the Discharge Coordinator Team in NUHT and other system partners, discharge hubs to identify and facilitate the discharge of patients from the acute hospital setting to community services.
4. Effectively liaise and maintain relationships with other system partners to ensure that appropriate equipment, POC, and support systems are put in place to meet the health and social care needs of patients. This may include restarts or new POC and equipment to support patients to return home or the provision of interim, step-down, or permanent residential placements.
5. Be an active participant in shift work, covering either 8-4, 9-5, 10-6, or 12-8 pm, with rota'd weekends and BHs as the service operates 365 days per year.

5. Act as a role model in the practice area, encouraging staff in teaching, supervision, facilitating and assessing, creating and sustaining a supportive/teaching/learning environment for the practice setting.
6. Be responsible for the maintenance and accurate recording of patient information in the documentation systems and ensure the effective dissemination of patient information throughout the multi-disciplinary team.
