Role: Interface Geriatric Consultant - substantive or 12 month fixed term contract. Visa sponsorship is available.
Rates: £99,532 to £131,964 per year + access to employee benefits, such as the NHS Pension Scheme
Shifts: Mon- Fri 0800-1600 + On call commitment OPAL Unit cover 1 in 6 Saturday + Sunday (9 hours) with day off in lieu. 8.5 DCCs & 1.5 SPAs. Full job description available on request.
The aim of the IGC is for early identification of patients attending the Emergency Department (ED) with a frailty score above 5 (Rockwood) to enable a multi-disciplinary team of frailty specialists (OPAL Outreach) to be mobilised that will support the assessment of an individual's needs, through a frailty specific lens, to identify and co-ordinate the sourcing of additional care needs in the right place and the right time, safely returning individuals back into community-led care plans as quickly as possible.
They are required to lead multidisciplinary team meetings and contribute to training programmes and service development. Where indicated they participate in and/or lead family meetings and attend relevant multidisciplinary team meetings within the hospital, for example the Nutrition MDT for complex nutritional issues.
The IGC role will encompass:
1. Attending the ED handover daily to identify frail, older patients for post taking
2. To see and screen patients within ED (to complete medical part of CGA)
3. To feed the most appropriate patients through to OPAL for assessment, overnight stay or referral to OPAL hot clinics
4. To support ambulance queue
5. To support AAU & SDEC
6. To support the GIFRT six steps for better care guidance
7. Support the achievement of the Ambulance national 30 minute category 2 target

Qualifications/Licenses/Certifications
Essential
8. Be a GMC registered Doctor (you do NOT need to be on the Specialist Register).
9. Have relevant experience at Consultant level, or references to support your transition up to Consultant level.
10. You will need to pass an enhanced background check (DBS).
