Could you be our new social worker? We are seeking a compassionate and dynamic new member of our multi-professional team to provide social work support to the inpatient unit, community team, and day services.

Hours: 30 hours per week
Salary: Up to £29,725.27 per annum
Overview of Role:
To work as a member of the multi-professional team providing social work support to the Inpatient Unit, community team, and Day Services including support for relatives and carers of Mountbatten’s patients.
To work with patients and families in a way that identifies their personal goals and to help tailor care from Mountbatten and external organisations to support these goals.
To support patients and their families within their own social context and cultural frameworks and advocate for them as and when required.
To act as the safeguarding lead and enable the wider MDT to identify and respond to safeguarding concerns. To deliver regular safeguarding training as part of the lead safeguarding role.
Undertake holistic assessments for patients, carers, and service users and ensure they are reviewed.
Key Tasks:
1. Promote equality as an integral part of the role and treat everyone with fairness and dignity.
2. Identify vulnerable people at risk of abuse and initiate safeguarding processes where appropriate.
3. Promote, enable, and empower individuals, families, and carers through representation of rights; advocacy; supporting patients and families to make informed choices about complex life decisions.
4. Facilitate and support patients and their families to undertake shared self-assessments and reviews to identify needs and outcomes.
5. Support patients and their families in the development of support plans using a person-centred approach in liaison with statutory and non-statutory/voluntary agencies.
6. Enable patients’ choice and empowerment through promoting independence as far as possible.
7. Facilitate patients/carers in the identification of services within the community and purchasing of services on their behalf.
8. Ensure a high level of data quality for all information recorded.
9. Maintain and contribute to the development of relevant records and statistical data.
10. Hold a caseload of a level of complexity and provide case management, guidance, support, and expert advice to the multi-disciplinary team.
11. Assess and manage risk to individuals, families, carers, groups, communities, self, and colleagues.
12. Understand and respond to ethnic, cultural, and religious influences of the patient and family/carers/friends.
13. Encourage, develop, and maintain effective working relationships with service colleagues and partner agencies.
14. Work as a member of multi-disciplinary teams providing expert opinions on the issues around the Care Act (2014) and Social Care.
15. Present patients at multidisciplinary meetings from individual caseload and on behalf of the team.
16. Negotiate with local authority services, community health care teams, local service providers, and voluntary organisations to meet the needs of patients being discharged.
17. Consult and work effectively with other professionals to identify and meet patient/carers needs.
18. Have a good working knowledge of the Continuing Health Care process and eligibility criteria.
19. Promote multi-disciplinary working partnerships.
20. Carry out assessment and discharge planning in accordance with the Care Act (2014).
Professional Registration and Standards:
1. Keep up to date with training and personal development to ensure professional registration with Social Work England is maintained.
2. Ensure professional practice is delivered within the boundaries of the Codes of Practice for Social Workers.
3. Stay updated with relevant legislation or changes in social policy that impact social work practice.
4. Manage and prioritise workloads using accountable professional judgment.
Audit, Research and Education:
1. Provide learning opportunities for student social workers.
2. Participate in education and provide advice to social work and health colleagues.
3. Adopt a reflective approach from personal practice and learn from experience.
4. Act as an educational resource.
5. Maintain knowledge of safeguarding policies and procedures.
6. Research and maintain a knowledge base of available resources.
7. Participate in the development of educational programmes.
8. Engage in audit, evaluation, and research as required.
9. Monitor performance against agreed objectives.
10. Provide training as part of the training programme for staff and volunteers.
Communication:
1. Effectively communicate with patients, carers, nursing staff, allied health professionals, and the wider multi-professional team.
2. Demonstrate advanced communication skills in complex and emotionally sensitive situations.
3. Respond to feedback from patients, families, carers, and colleagues.
4. Ensure effective record keeping and documentation.
General:
