Job summary We are looking for an enthusiastic, motivated, and compassionate individual to join us in the Community Stroke Team in Nottingham City. The CityCare community stroke team is a large multi-disciplinary team that consists of specialist nurses, therapists and support staff. The team acts within CityCares organisational values and behaviours making for a great working environment. You will be supporting individuals, their families and carers in their own homes and other community settings such as care homes. This is a people focussed role, seeing clients from within Nottingham Citys diverse population. This includes assisting patients who are attending rehabilitation groups such as Stroke Ability and Chat Ability. The role itself underpins the service by carrying out interventions and care plans established by specialist nurses, therapists and additional clinical staff. Competency based training is a requirement of the role to support the successful candidate. The service operates 7 days a week so some weekend work on a rota basis is a requirement of the post. However, this includes daytime work only. Main duties of the job The post holder will be involved in providing rehabilitation by following goals set by clinicians within the team to optimise the patients level of independence. The Rehabilitation Support Worker will promote independence using skills from Occupational Therapy, Physiotherapy, Speech and Language Therapy and Nursing, following a competency-based framework. A background of caring for people is essential either in an informal capacity or in a paid role. In addition to clinical skills, you will need to demonstrate excellent interpersonal and communication skills. You must be able to prioritise tasks and manage your time effectively. You will need to be flexible, innovative, organised, and keen to learn for the benefit of your patients.
