Filey And Scarborough Healthier Communities Network is seeking a PCN Nurse to join our proactive, multi-disciplinary team dedicated to enhancing the health and well-being of our patient population. As a PCN Nurse, you will play a crucial role in delivering evidence-based nursing services, with a primary focus on health promotion, chronic disease management, and health prevention within the Proactive Care Team.
Main duties of the job
As a PCN Nurse with us, you will:
1. Develop, implement, and embed health promotion and wellbeing programs.
2. Create and evaluate individual treatment plans for both acute and non-acute conditions.
3. Support the development of individual treatment plans for patients with chronic diseases.
4. Identify and manage patients at risk of long-term conditions, preventing adverse health effects.
5. Provide routine nursing care in accordance with clinical-based evidence, NICE, and the National Standards Framework.
6. Prescribe medication within your scope of practice, adhering to guidelines and protocols.
7. Review medication for effectiveness and ensure alignment with national guidance.
8. Request and provide advice on prescribed or over-the-counter medication.
9. Support patients in compliance with prescribed medications.
10. Request pathology services as necessary and interpret results.
11. Refer patients to other services when appropriate.
12. Maintain accurate clinical records / personal care plans, using effective SNOMED codes.
13. Maintain chronic disease registers.
14. Prioritise health issues and intervene appropriately.
15. Recognise, assess, and refer patients with mental health and community health needs.
16. Implement vaccination programs for adults and children.
17. Liaise with external services/agencies to ensure vulnerable patients are supported appropriately.

The PCN Nurse will form an integral part of the PCN Proactive Care Team working alongside the Clinical Director, ARRS role staff, Practice Managers, partner organisations and local communities, to ensure the delivery of the Primary Care Network priorities.
Job responsibilities
Job Summary
The post holder will be responsible for the delivery of evidence-based nursing services, working as part of the proactive, multi-disciplinary team, delivering care within their scope of practice to the entitled patient population.
Working autonomously, you will be responsible for a number of clinical areas such as health promotion, chronic disease management and health prevention, as well as supporting the management team in the review of clinical policy and procedure.
Your primary focus will be to support the delivery of the PCN Network DES, Enhanced Health in Care Home framework and achievement of the Quality Outcomes Framework, working within your professional competencies. To do this you will be expected to visit patients in their care home, own homes or the Practices.
Additional training will be offered, such as minor illness training and Non Medical Prescribing as part of the career development opportunities that this role offers.
Primary Responsibilities
1. Develop, implement and embed health promotion and wellbeing programmes.

9. Support patients to ensure compliance with prescribed medications.
10. Request pathology services as necessary, processing and interpreting results as required.

12. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
13. Maintain accurate clinical records in conjunction with extant legislation.
14. Ensure SNOMED codes are used effectively.

16. Prioritise health issues and intervene appropriately.
17. Support the team in dealing with clinical emergencies.
18. Recognise, assess and refer patients presenting with mental health needs.
19. Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways.

21. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.).
22. Support the clinical team with all safeguarding matters, in accordance with local and national policies.
23. Understand organisation and local policies for substance abuse and addictive behaviour, referring patients appropriately.
