Barts Health NHS Trust is looking for an enthusiastic and proactive individual to provide operational leadership and patient case management within the Transfer of Care Hub Team.
The successful individual will be part of a multidisciplinary team consisting of nursing, therapy, social work, discharge navigator and admin team members. You will be working closely with health and social care partners to ensure safe and timely discharges and minimise barriers, improving patient experience across discharge pathways.
You will have close links with community services, delivering a discharge to assess model of care in the community. You will be supported by the Transfer of Care Hub manager and work closely with system partners including acute staff, local authorities, voluntary agencies and the ICB.
The post holder will be an active participant in shift work, working 8am-4pm Monday to Friday but also part of a rota covering some late shifts up to 8pm and rota’d weekends and Bank Holidays as the service operates 365 days per year.
The post holder will manage and provide leadership to the Transfer Of Care Hub at The
Royal London Hospital. The post holder will coordinate the discharge process for patients with complex discharge needs from admission .This will require the post holder to possess expert communication and negotiation skills with patients, carers, internal and external stakeholders to minimize system delays and support the Division’s to reach their length of stay standards.
The post holder will work with internal and external stakeholders to develop
communication and referral pathways which aid the discharge process for all members of the multi-disciplinary team.
The service operates 7 days a week from 08:00 to 20:00 including bank holidays. Usual working pattern is Monday to Friday 8am-4pm. If successful, please note you will also be expected to work on rota’d weekends, bank holidays and late shifts as required by the service.
