MAIN TASKS AND RESPONSIBILITIES
1. Supporting service users to maximise their independence and well-being through facilitating access to a range of internal and external vocational, learning and recreational opportunities.
2. To work in conjunction with relevant colleagues to develop and deliver bespoke vocational, learning and recreational programmes to meet individuals' identified outcomes and goals.
3. To develop and deliver group sessions within the service that meet service users identified outcomes.
4. To develop partnerships with external vocational/learning and recreational providers to maximise vocational/learning/recreational opportunities for individuals both within and external to the service.
5. To contribute to the development of Service User goals and outcomes in relation to vocational, learning and recreational opportunities.
6. To identify and organise and co-ordinate a range of activities to meet the rehabilitation goals and personal interests of service users
7. Undertaking risk assessments for vocational/learning/recreational activities identified
8. Supporting service users in participating vocational/learning/recreational activities
9. Design, implement and lead, in conjunction with others from the clinical team, individual and group based activities which support rehabilitation goals
10. To contribute to reviews and other professional and team meetings, ensuring accurate record keeping of service user progress
11. To contribute to goal setting and rehabilitation review reports as appropriate.
12. As part of the development of the service, the post holder will be expected to contribute ideas to a the service development plan and capital expenditure items
13. Establish and maintain professional working relationships with the service users, contributing to the creation of a homely and pleasant living environment for each service user
About you
14. Working within a multidisciplinary environment or experience of devising and implementing multiple activity programmes
15. Understanding of health and safety issues in the design and access of activities.
16. IT skills including the ability to open and send emails using Outlook, the ability to use Microsoft office and the ability to navigate the internet
17. Excellent communication skills both verbal and written forms
18. Full driving license and ability to drive the service vehicles
