Anopportunity has arisen to join the Locality Coordinator team at Your Healthcareas a Care Home MDT Coordinator. TheLocality Coordinator Team coordinates and administrates the monthly andfortnightly GP practice-based MDT meetings at each of the practices across bothKingston and Richmond. This Care Home MDT Coordinator position is vital to theroll out of a proof of concept, delivering MDTs in select practices in Kingstonand Richmond.
Weare looking for an enthusiastic individual who can demonstrate initiative, professionalism,and a strong team-working ethic. The successful candidate will be able to workindependently when required, work effectively with a wide range of health andsocial care professionals, and contribute to improving MDT working as part ofthe team.
Main duties of the job
Theproof of concept to deliver MDTs in Care Homes has been modelled in line withthe Proactive Anticipatory Care (PAC) Programme which ran as a successful proofof concept for two years, followed by a borough-wide roll out in September2023.
Thepost will be fundamental in supporting the Care Home MDT meetings to functioneffectively, implement changes to ways of working and evaluate the success ofthe model. The post holder will also maintain a wide range of administrativetasks to a high level and keep up with the changing demands of the service.
Itis essential that candidates have both experience of and interest inadministration and MDT working. You will have excellent written and verbalcommunication skills and good attention to detail. You will be competent andconfident in the use of IT systems/platforms used within Your Healthcare. Job responsibilities
The post holder/s will support the ongoing implementationof Integrated Neighbourhood Teams & MDT working in Care Homes across theRoyal Borough of Kingston & the borough of Richmond. The postholder will befundamental in supporting Primary Care Networks (PCNs) to function effectively,implement changes to ways of working and evaluate the success of this new wayof working.
The postholder will support the administration, implementation,management, and delivery of enhanced MDT working in care homes across Kingstonand/or Richmond.
The teams who deliver Care Home MDTs are made up ofprofessionals from many different services, such as GP Practices, Social Care,Secondary Care and community nursing. Enhanced MDT working in Care Homes aim toimprove the use of the health and care system by intervening earlier,proactively and more holistically. The post holder will be responsible forliaising with all members of the MDT, in addition to wider specialists, toensure the meetings run smoothly, and referrals are managed effectively.
Thepost holder will have a key responsibility to shape the requirements of thisrole in the future. Whilst the job description and person specification outlinekey attributes, the post holder will need to be flexible and open to changingand developing the role as required.
Key Result Areas
Supportingthe functioning of the care home multidisciplinary team (MDT)
Workingin partnership with the PAC locality coordinator/manager to share informationand identify care home residents for MDT discussion.
Liaise with ClinicalCare Practitioner to identify care home residents for MDT discussion and ensurethe right professionals are involved, notified, and prepared.
Producesummary notes of MDT discussions/meetings including agreed actions.
Informationgathering through coordinating professional input and outputs from the quarterlyrisk-stratification tool using the local case-management system to access key information.
Participatein the review of patient outcomes and goals in keeping with the requirements ofthe role.
Alongsidethe Clinical Care Practitioner act as a contact point for all Care Home MDTsthat you will be supporting.
Inconjunction with the PAC team and wider stakeholders, continually work toreview and improve processes to ensure MDTs function in the most effective way.
Monitor keyactions and highlight review periods set by MDTs.
Support thedevelopment of Integrated Neighbourhood Teams (INTs) within Kingston andRichmond. This will involve working closely with PCNs to support their growthas an INT
Working inpartnership with the Urgent Care & Support Service in Kingston and CareHome Support Team in Richmond, monitoring hospital admissions for care homeresidents, supporting the Clinical Care Practitioner to enable proactiveworking.
Information management,analysis and evaluation
Establishagreed performance and reporting systems and undertake case tracking ofindividuals discussed to support the proof-of-concept evaluation
Working withthe MDT to distribute, process and analyse wellbeing surveys to patients andusers to support the proof-of-concept evaluation
Accessrequired to organisational information systems for information gathering andinputting as required to support MDTs.
Communication and workingwith others
Establishgood working relationships across a range of professionals withinmulti-disciplinary teams including health, social care and voluntary sectorstaff.
Liaise with the core MDT membership, the HIU team, andLAS, alongside other service providers, to identify patients suitable forreferral to MDTs in Care Homes
Provide admin support to the MDT to set uphuddles as required.
Engage with PACQuality Improvement (QI) meetings as appropriate to share good practice.
Present andcommunicate information in a range of formats to support engagement.
Report any issues or updates to the programme teamin a timely manner.
Roledevelopment
Work withthe wider team and programme manager to develop the role, requiredresponsibilities, functions and requirements going forward.
