To provide non-medical led clinical management for patients requiring care across a number of specialities in the community.
The trainee advanced clinical practitioner (TACP) will work within a defined scope of practice and to agreed clinical standards applied within a Trust wide and local setting. The role will be multifactorial and include non-medical prescribing, advanced clinical assessment of all systems, diagnostic skills and understanding of investigations. The role will improve the quality of, and access to, health care provision and will strengthen professional leadership.
The trainee advanced clinical practitioner will only provide clinical care for adult patients.
· Manage own case load of patients, developing and agreeing management regimes with the patient that acknowledge, and are responsive to their clinical, psychological, physiological, and educational needs.
· Demonstrate judgment in assessing wide ranging and complex patient problems. Agree solutions, ensure, and promote options to enable the delivery of optimum patient care.
· Maintain the identified specialist competencies of a Trainee Advanced Clinical Practitioner
· Responsibility for the assessment of care needs, the development and evaluation of programmes of care and the setting of standards of care aimed at providing a high quality of service
· To utilize clinical skills and knowledge to instigate changes to treatment regimes, and to undertake the role of the patient’s advocate when appropriate.
· Within agreed protocols order investigations and prescribe drugs as required (on completion of the non-medical prescribing module)according to agreed protocols.
